  1. Release of Liability: In consideration of permission to use and participate in the Got ROM, Inc VIP Mentorship Program, I, for myself and on behalf of my executors, administrators, heirs, and assigns, do hereby expressly release, discharge, waive, relinquish, and covenant not to sue Got ROM, Inc. and/or the Trainer/Massage Therapist, their affiliates, officers, directors, agents, or employees from any claims, demands, injuries, damages, causes of action, or liability in personal injury, accidents, illness (including death), and property loss arising from my participation in, observation of, or use of the program's property, facilities, staff, equipment, and/or services. This release applies to claims arising from negligence, whether directly or indirectly.
  2. Privacy and Data Security: While Got ROM, Inc. has made efforts to protect privacy and security of data, I release them from any claims, demands, injuries, damages, causes of action, or liability, including negligence, arising from the breach of Got ROM, Inc. servers.
  3. No Medical Services Provided: I understand that Got ROM, Inc. does not provide medical services, diagnoses, or treatments.
 
ACCEPTANCE OF RISK AND ACCEPTANCE OF RESPONSIBILITY
 
I acknowledge that the Got ROM, Inc VIP Mentorship Program involves certain inherent risks that cannot be eliminated. The program may include activities such as massage, instrument-assisted tissue work, stretching, weight lifting, running, aerobics, circuit training, and/or strength training. These activities may involve strenuous exertions of strength, quick movements, and sustained physical activity, which may stress the cardiovascular system.
 
The specific risks associated with the activities may vary, depending on the activity and individual circumstances. These risks can range from minor injuries like scratches, bruises, and sprains, to major injuries such as eye injury or loss of sight, joint or back injuries, heart attacks, concussions, and catastrophic injuries including paralysis and death.
 
If I have a known heart condition, experience chest pain when exercising, or take medication for high blood pressure or a heart condition, I confirm that I have been cleared by my physician for exercise.
 
I have read and understood the above risks, and I voluntarily assume all such risks associated with participating in the Got ROM, Inc VIP Mentorship Program.
 
GENERAL TERMS
 
The parties hereto agree that this agreement shall constitute the entire agreement between the parties for the subject matter contained herein, together with any agreement related to the amount of fees charged by Got ROM, Inc. This agreement may be executed electronically or via facsimile and in counterparts. The agreement is governed by Michigan law, without respect to choice of law provisions. Any disputes between the parties shall be resolved in Washtenaw County, Michigan.
 
ACKNOWLEDGEMENT OF UNDERSTANDING
 
I have read this liability waiver, assumption of risk, indemnity agreement, and fully understand its terms. I acknowledge that I am signing this agreement freely and voluntarily, and intend for my acceptance by checking the designated box to be a complete and unconditional release of all liability to the greatest extent allowed by law.
